Job Summary

The purpose of the Medication History Technician is to independently obtain, review, clarify, document and verify with the patient and/or family, their home/current medication list for patients that will be admitted to the hospital from the Emergency Department. The Medication History Technician will utilize the patient’s pharmacy, family, caregiver, or prior office records if needed to further investigate and verify medications on, to be added or to be removed from the home/current medication list. The technician will keep records of all work done for medication reconciliation performance monitoring.

Job Requirements

  • Education
    • High School Diploma or Equivalent
    • Preferred: Associates Degree in a Related Field
  • Experience
    • 1-2 Years Retail or hospital pharmacy experience
  • Skills
    • Interpersonal skills. Ability to interview patients and interact with healthcare professionals.
  • Licensure/Certification/Registration
    • Required: Registered with the with the Missouri State Board of Pharmacy as a pharmacy technician or application completed upon hire and obtained within 90 days of hire.
    • Required: Certified Pharmacy Technician must be obtained within 6 months of hire.
