Home health care can include skilled nursing and therapy services in your home. When your doctor orders it, services may also include a home health aide, occupational therapy, or a medical social worker as part of your care plan.

In most cases, you’ll need a doctor (or other qualified provider) to order home health services. Medicare coverage generally requires part-time or intermittent skilled care and the patient must be “homebound,” meaning leaving home is difficult and requires major effort.

Start by calling CoxHealth at Home to talk with a Care Coordinator or by asking your doctor to refer you. After a referral, the home-health agency schedules a first visit to talk about your needs and coordinate a care plan. The timing depends on clinical need, insurance approvals, and scheduling.

Visit frequency is based on what your provider orders and what your care plan requires. For Medicare-covered home health, services are typically part-time or intermittent, not around-the-clock.

Palliative care focuses on relief from symptoms, pain, and stress for people living with serious illness and may be provided alongside treatment. Hospice care usually begins when a patient has a life expectancy of six months or less and chooses not to pursue curative treatment.

Hospice care is generally appropriate when a doctor certifies a terminal illness and life expectancy is about six months or less (if the illness runs its normal course without the patient choosing to seek curative treatment). “End-of-life care” means help managing symptoms and stress so a person can live as fully and comfortably as possible.

Yes. CoxHealth at Home offers bereavement services such as seminars, support groups, and staff follow-up, and hospice regulations include bereavement counseling as part of hospice care.

CoxHealth at Home is available to help answer questions 24 hours a day, every day of the year. For hospice care, nursing services and related support, such as physician services and symptom-management medications, must be available on a 24-hour basis, 7 days a week.

CoxHealth at Home works with Medicare, Medicaid, and many private insurance plans, and our team can help you understand your coverage. Medicare may cover skilled visits when they’re ordered by your doctor, and Medicaid coverage depends on eligibility. Hospice is covered under Medicare when the hospice eligibility requirements are met.
