Remote Patient Monitoring for Chronic Disease Management

How Remote Patient Monitoring (RPM) Works
CoxHealth at Home provides easy-to-use connected medical devices that automatically collect and transmit your health data to our clinical team. Depending on your condition, you'll receive devices such as:
- Blood pressure cuff
- Digital weight scale
- Pulse oximeter (oxygen levels)
- Blood glucose meter
While telehealth encompasses various remote care services including video appointments, remote patient monitoring uses connected medical devices to track your vital signs between appointments. Looking for video consultations? Visit our Virtual Visits page.
Daily Monitoring Process
Each day, you'll take your measurements using the provided devices. The information automatically transmits to our trained clinical professionals who review your data according to guidelines established by your doctor. When concerning trends are identified, our team responds proactively, communicating with you and your physician before small changes become serious complications.
Easy Setup & Support
One of our experienced CoxHealth at Home staff members will come to your home to set up your RPM system, demonstrate how the devices work, and answer any questions. Our remote patient monitoring call center is accredited as a Health Call Center by URAC, and we offer the only locally managed RPM program in our area.
