CoxWorks Request for Removal Form

• This form should be completed by authors who wish to have their work removed from the CoxHealth institutional repository, CoxWorks.
• This form must be submitted by all authors for a work to be removed. If authors are not in agreement, requests may also be submitted for review to the CoxWorks Institutional Repository Advisory Council.
• Upon submission, a confirmation email will be sent to each contributing author at his/her CoxHealth or Cox College email address to authenticate submission. Affiliates who do not have a Cox email account should use their individual email account issued by the affiliated institution. (e.g. MSU, MU, CDC, etc.) If you are no longer affiliated with the same institution, please use the individual email account issued by your current employer or educational institution. This is to ensure the legitimacy of the request, but exceptions may be made on a case by case basis.

Removed Items

• The record for the item will be suppressed in the EOS catalog so it is no longer visible or accessible on the open web.
• Works will be perpetually maintain in a designated folder on the Library Services H:/ and only accessible to Library Services staff and appropriate administrators.
• Authors wishing to have their works removed from EBSCO Open Dissertations should contact EBSCO.
