ReadySet Setup Instructions

Complete the following steps to create your account:

1. Create your ReadySet account

   1. Go to the ReadySet web page
   2. Click “Create a new account” and follow the instructions. You will need the following information:
      1. Organizational 4-digit Code: 1807
      2. Choose the appropriate Program Type:
         1. New Hire
         2. New Hire Food Service (Food Service Employees Only)
         3. New Hire Volunteer
      3. Population type:
         1. Employees choose: Post Offer Candidate
            1. Do NOT choose “Employee” or the system will block you from creating an account.
         2. Volunteers choose: Volunteer

2. Complete the TB Program Symptom and Exposure Questionnaire

   You will see a red icon next to View Surveys. Click on this and complete your TB Questionnaire. You will have a green checkmark under status when you are finished.

3. Complete the OSHA Respiratory Survey (if applicable)

   If an OSHA survey is required you will see a red icon next to View Surveys. Click on this and complete your survey. Your survey will have a green checkmark under status when you are finished.

4. Upload your vaccination records using the instructions below

   If you do not have records of your vaccination, see below for where they can be obtained. Failure to provide required documentation may lead to administration of additional vaccinations.
